Our pharmacists have expertise in the delivery of patient notes review and can provide support to improve the management of patients with, or at risk of, malnutrition to ensure they are managed in line with best practice guidelines.

Our pharmacists will undertake a comprehensive clinical assessment of patients’ notes considering current clinical condition and indication for ONS, concordance with treatment and attainment of treatment goals.

They can make recommendations for ongoing treatment, or for further assessment or referral to secondary care where appropriate.

Using your practice clinical system and specially designed searches we can:

  • identify cohorts of patients for review
  • provide a baseline report to give you an overview of current oral nutritional supplement (ONS) prescribing
  • identify patients at risk of malnutrition for nutritional screening and education
  • identify patients currently being prescribed ONS outside of the practice preferred pathway and current guidelines

Our pharmacists can facilitate the invitation of high-priority patients for practice review and provide educational materials to help patients understand their risk of malnutrition and make appropriate changes to their diet.

In addition, the pharmacist can provide educational support to practice staff, and you will be provided with a report to demonstrate Quality Improvement in the management of patients with, or at risk of, malnutrition.


The review conducted by Interface was extremely thorough and beneficial to our patients.  As well as the clinical benefits associated with improved patient care, the Interface pharmacist was able to reduce our prescribing costs helping the NHS save money. This will be of great assistance to our practice and the CCG going forward.
