      On average, malnourished individuals see their GP twice as often, have three times the number of hospital admissions and increased duration of hospital stays.1 It is estimated that £5000 could be saved per patient per year through better nutrition management and reduce the burden on the NHS.

      Using your practice clinical system and specially designed searches Interface can:

      • Identify cohorts of patients for review
      • Assess ongoing individual patient need, compliance and treatment goals
      • Improve quality of prescribing and adherence to local guidelines
      • Improve patient understanding of ONS and food fortification guidance
      • Reduce waste and potential ONS expenditure through alignment to practice specified ONS formulary

      Our pharmacists can facilitate the invitation of high-priority patients for practice review and provide educational materials to help patients understand their risk of malnutrition and make appropriate changes to their diet.

      In addition the practice will be provided with a report to demonstrate Quality Improvement in the management of patients with, or at risk of, malnutrition.

      The review conducted by Interface was extremely thorough and beneficial to our patients. As well as the clinical benefits associated with improved patient care, the Interface pharmacist was able to reduce our prescribing costs helping the NHS save money. This will be of great assistance to our practice and the CCG going forward.
