Attend 2 Bone Health
The Attend2 Fracture clinical toolkit enables GP practices to undertake a proactive fracture risk assessment (FRAX) for all recommended patients (NICE CG146) to identify those at risk of an osteoporotic fracture.
Attend2 Fracture will help to prioritise cohorts for review and also support clinicians in making decisions for individual patients by providing clinical data relevant to their bone health. Attend2 Fracture facilitates the identification of patients receiving bone sparing therapy with comprehensive information provided relating to compliance, treatment failures and other clinical issues.
Attend2 Fracture includes:
As well as access to the Attend2 Fracture, practices will receive on-site support form an Interface clinical pharmacist, to ensure clinicians achieve the most out of the toolkit. This includes identifying high-risk patient cohorts and implementing pro-active and effective workstreams and preventative strategies. Attend2 Fracture also includes a revalidation report to highlight the progress made within the practice and identify any areas where further improvements could be made.
Excellent service from the Interface pharmacist who went through our patients methodically, with minimum disruption and kept me informed at every step of the way. He was patient in his explanations of things to me and was highly knowledgeable. I would thoroughly recommend this service.
Interface takes the spread of COVID-19 extremely seriously. Our priority is the safety of patients, employees, NHS staff and their families. We’re adapting our services to deliver clinical support remotely including the patient clinics incorporated within many of our reviews.Learn more
