Changes to QOF CVD Indicators: What Practices Need to Know

The 2025/26 QOF update marks a significant change to QOF CVD Indicators in how cardiovascular disease (CVD) is managed and incentivised in primary care. With NHS England placing CVD prevention at the heart of its strategy, practices and GPs must adapt quickly to meet new expectations—and unlock substantial funding.

What’s Changed?

 

  • Retirement of 32 Indicators: These have been replaced or consolidated to streamline focus.
  • £198 Million Reallocated: A total of 141 QOF points have been reallocated to prioritise the prevention of cardiovascular disease, aligning with NHS strategic objectives.
  • Raised Performance Thresholds: Upper thresholds now reach 85–90%, demanding near-universal coverage or robust exception coding.

  • Proactive Management Is Essential: Practices must intensify recall systems, structured reviews, and medication optimisation.
  • Data Accuracy Matters: Proper coding and exception reporting will be critical to achieving targets and securing funding.
  • Team-Based Care: Pharmacist-led reviews, home BP monitoring, and shared decision-making are central to achieving CVD Indicators
  • Training & Workflow Updates: Clinical teams need to be upskilled on new thresholds and treatment protocols.
