The project, Supporting the management of type 2 diabetes with pharmacist-led reviews: an observational study, was delivered in Slough CCG and focused on improving the management and control of type 2 diabetes (T2D) using the 9 key care processes.
Interface pharmacists reviewed 5,910 patients with T2D which resulted in an increase in patients receiving all of the NICE-recommended 9 key care processes whilst the number of patients achieving glycated haemoglobin (HbA1c), blood pressure (BP) and total cholesterol (TC) targets also increased.
