This will ensure more patients are prevented from having cardiovascular disease, heart attacks and strokes.​

IMPACT CVD Prevalence

QOF payments for 25/26 will continue to be heavily influenced by CVD recorded prevalence, it is ​imperative that practices  and PCNs maximise this funding potential by ensuring as many eligible patients are on CVD registers as possible.​

IMPACT CVD Prevalence includes:

  • QOF Disease Prevalence tool covering all CVD domains:

AF, HF, CHD, PAD, S/TIA and Hypertension

 

 

 

 

  • The HF plus tool
  • 2 pharmacist-led implementation or consulting days
  • 1 revalidation
