IMPACT CVD Prevention

CVD IMPACT can help practice achieve key Investment and Impact Fund (IIF) indicators.  NHS England aims to prevent 15,000 heart attacks and strokes by 2029.  The key to avoiding cardiovascular events is early detection and intervention.  To support this aim, funding has been made available to PCNs through the Investment and Impact Fund (IIF).  An average PCN can earn £30,000 by achieving all of the cardiovascular indicators.

However, achieving these indicators, is a complex process.  PCNs will need to navigate a complex set of business rules and identify 1000s of eligible patients to ensure they are appropriately managed.  This is where IMPACT CVD can help.

CVD IMPACT with consultancy day   £2175 + VAT

  • CVD IMPACT Prevention platform, identify patients at risk of CVD and achieve IIF indicators.
  • User guide to help understand the tool and how best to implement it within the practice.
  • One consultancy day,  delivered by a Clinical Pharmacist. Training will be provided on how best to use IMPACT CVD to benefit patients and work towards the IIFF indicators.  The pharmacist will support practice to work through patients and immediately deliver clinical outcomes.

Data refresh   £100 + VAT

  • An updated IMPACT CVD platform housing refreshed data to help deliver continued clinical outcomes.
  • A maximum of 4 data refreshes can be delivered across the year.
