IMPACT CVD Prevention

CVD IMPACT can help practice achieve key Investment and Impact Fund (IIF) indicators.  NHS England aims to prevent 15,000 heart attacks and strokes by 2029.  The key to avoiding cardiovascular events is early detection and intervention.  To support this aim, funding has been made available to PCNs through the Investment and Impact Fund (IIF).  An average PCN can earn £30,000 by achieving all of the cardiovascular indicators.

However, achieving these indicators, is a complex process.  PCNs will need to navigate a complex set of business rules and identify 1000s of eligible patients to ensure they are appropriately managed.  This is where IMPACT CVD can help.
